If you are already a client of Bulimba Vet Surgery, please complete this form.

 

You will then be sent a patient intake form to complete. We also request that you contact your vets to get any relevant history or x-ray imaging sent through to info@reboundpetrehab.com. Once we have received these, one of our nurses will contact you to make an appointment.
